All applicants:

• Application form
  • Did you write your field of interest?
  • Did you complete the back of the form?
• Photo tickets A and B, examination admission ticket
  • Did you attach your photos?
• Payment certificate (see the Guidelines for Applicants for payment instructions)
  • Is the payment certificate for 30,000 yen attached in the appropriate space?
    Note: Not required if you are expecting to obtain a Master’s degree from The University of Tokyo or if you are a foreign applicant on a MEXT scholarship. If you are the latter and not a current student or research student of the university, submit proof of the scholarship instead.
• Academic transcript
  • Did you enclose the grades in your major?
  • Did you enclose your undergraduate transcript?
  • Did you enclose a transcript of your Master’s program?
    Note: Not required if you have completed or are expecting to complete a Master’s program of the Graduate School of Frontier Sciences.
• Graduation certificate
  • If you have completed a graduate program, is your graduation certificate enclosed?
    Note 1: Proof of expected graduation is not required.
    Note 2: Not required if you have completed a Master’s program of the Graduate School of Frontier Sciences.
• Self-addressed return envelope
  • Did you write your name and address, and affix the correct postage (430 yen)?
• Address label
  • Did you write the correct address to mail correspondence regarding the acceptance notice and enrollment forms?
• Admission questionnaire
  • Did you select the correct version for the Doctoral Program?
  • Did you indicate your preferred thesis advisor?
• Work–study balance plan
  • I work at a company, government agency, or other organization, and intend to enroll while keeping my position.
